New Student Registration

Registration materials are available below as well as in the Golden Brook School foyer. We suggest that you download, complete, and print the registration form and bring it with you along with the paperwork outlined below.

In order to be eligible for Kindergarten, children must be born between October 1, 2011 and September 30, 2012. Kindergarten students must be age 5 by Sept. 30, 2017. (Per RSA 193:1 – any child at least 6 years of age shall attend full-time education). Any student born before October 1, 2011 will be enrolled in first grade.

In order to be eligible for First Grade, children must be born between October 1, 2010 and September 30, 2011. First grade students must be age 6 by Sept. 30, 2017. (Per RSA 193:1 – any child at least 6 years of age shall attend full-time education).

In order to be eligible for Second Grade, children must be born between October 1, 2011 and September 30, 2010. Second grade students must be age 7 by Sept. 30, 2017. (Per RSA 193:1, any child at least 6 years of age shall attend full-time education).

Required Items Due at the Time of Registration for all Students: 

COMPLETED registration packet includes the following items. Please bring these items to one of the registration days.

  1. Completed Registration Form.
  2. Original Birth Certificate with raised seal; copies will not be accepted.
  3. Immunization Records with updated vaccinations. Registration cannot proceed if immunizations are not up-to-date or missing! (Please check with the doctor’s office prior to registration day.)
  4. Valid Driver’s License with current address. (Obtain a valid NH license if moving from another state within 60 days of closing, or 30 days from moving from another NH town).
  5. Current Utility Bill (i.e. electric/gas, mortgage, cable or, closing documentation/leases also acceptable).
  6. For Purchase and Sales Agreements, please see WSD Policy concerningAnticipated Occupancies - JFAB-R Procedures for Non-Residents who are relocating to Windham and JFFA Admission of Resident Students. A letter to the superintendent is required if a Purchase and Sales Agreement is the only proof of anticipated residency. Please send letter concerning your anticipated residency to the SAU #95 Superintendent’s office with a copy of your P&S. Please bring a copy of the letter when registering your student.
  7. See attached affidavit required if living under other special circumstances in Windham. 
  8. Physical Exam dated September 1, 2016 or later (A recent physical exam form will be accepted, if your child has a physical scheduled between January 2, 2017 and September 1, 2018. You must provide the date of the upcoming exam to the school nurse and the updated form must be submitted before school begins.)
  9. Authorization for Release of Student Records - MUST be signed by parent or legal guardian.

The Windham School District uses an online-based system called InfoSnap for collecting student information, which includes emergency contacts, medical history, parent questionnaires, etc. After in-school registration is complete, InfoSnap will send an email directing you to the on-line registration portion. It is important that you provide a valid email address. Please check your email for the link in the weeks following registration.
